Walsh has special needs and other mental disabilities, so he lives at the Pleasant Valley Nursing Center in Derry. Two of Walsh's sisters were the last ones to see him when they took him out to eat.

Jennifer Bakanosky, one of Walsh's sisters, said that they signed him back into the nursing home at a quarter of 1 p.m. He then had a doctor's appointment at the nearby Parkland Medical Center, where he was transported by nursing center staff.

Hospital surveillance video shows Walsh leaving the hospital alone, and he has been missing ever since.

Walsh was last seen wearing a brown jacket, gray t-shirt, blue jeans and white sneakers. He's 63-years-old and approximately 6-feet-tall. The family said he most likely has facial hair at this point.

A Facebook page called Bring Jack Home has been made for people to provide updates about the search.

The Derry Police Department is actively searching for Walsh, if located or seen, please contact the Derry Police Department at (603) 432-6111.
